6. What is the purpose and how does the legislatively directed spending item provide a public benefit that is an appropriate use of taxpayer money?
Authority Health seeks an appropriation to create connection between young to super seniors and a primary care, including improved access through group trainings and 1:1 coaching, understanding how to navigate insurance eligibility, increased use of technology and insurance enrollment and health literacy. The goal is to provide individual care plans for a cohort or seniors to enhance focus on better control of diabetes, hypertension and early kidney detection. A reasonable estimate is that 10%-20% of seniors in Detroit may not have a primary care provider. Assuming 80,000 to 100,000 (65+) live in Detroit, the potential population to be reached is anywhere from 12,000 to 15,000. This population and the greater community will benefit with overall education and increased health outcomes. In addition, we anticipate a cohort of 200-250 seniors would receive care coordination annually. Success will be measured by demonstrated health literacy increased engagement with primary care physicians increasing impactful education in the community.
