  The funds will be used to increase access to health services in the Black Community. We will be partnering with Choose Healthy Life, a national non profit who advances a proven community centered health model through trusted leadership of the Black church. By embedding trained Health Navigators within faith based institutions, connecting medical expertise to communities where trust already exists. The requested amount is intended for $1.3million in current FY26 and $2m per year over three years beginning FY 27.
